Chronic Pelvic Pain Treatment for Men in Central Iowa

Man Stretching Indoors

Pelvic pain that won't go away wears on more than your body. It affects how you sit, work, sleep, and connect with the people closest to you, and it's exhausting when no one can tell you why it's happening.

At Revive Physical Therapy, our specialized pelvic health therapists treat chronic pelvic pain syndrome (CPPS) in men by finding what's driving your pain, not just chasing symptoms. We build a personalized plan to calm the pain, restore normal pelvic floor function, and help you get back in control.

You may experience some of the following:

Pain in the pelvis, groin, tailbone, low back, or lower abdomen

Testicular, penile, or groin pain; along the perineum (between scrotum & anus)

Pain during or after ejaculation

Pain that worsens with sitting

Urinary frequency, urgency, or burning with no infection found

A feeling of pressure, fullness, or tightness in the pelvis

Tension or tightness in the pelvic floor muscles

What is Chronic Pelvic Pain Syndrome?

Chronic pelvic pain syndrome, sometimes called chronic prostatitis or CP/CPPS, is pelvic pain in men lasting three months or longer without a bacterial infection to explain it. Many men are first told they have prostatitis and given rounds of antibiotics, only to find the pain never really goes away.

That's because in most cases, the problem isn't infection. It's often the pelvic floor muscles themselves: tight, overworked, and unable to relax. Tense muscles can irritate nearby nerves and create pain that spreads to the genitals, perineum, bladder, and low back. Stress and a sensitized nervous system can keep that cycle going.

The good news? Because muscles and nerves play such a big role, CPPS often responds well to specialized pelvic floor physical therapy, frequently without surgery or more medication, and with a plan built entirely around you.

Many Factors Contribute to Chronic Pelvic Pain

Chronic pelvic pain in men rarely has a single cause. It usually builds over time from a mix of physical habits, past injuries, and how your body holds stress. Understanding what's behind your pain is the first step toward treatment that actually works.

Chronic stress & muscle guarding

Past prostatitis or urinary infections

Breath-holding or bracing during lifting

Constipation or straining

Prolonged sitting or desk work

Prior surgery (hernia, prostate)

Cycling or repetitive strain

Nerve irritation or sensitization

Personalized Care

Our therapists are trained specifically in pelvic floor rehabilitation for men, a specialty most clinics don't offer and most men don't know exists. That matters with CPPS, where generic stretching or strengthening can make symptoms worse. Your plan is built around your body, your pain, and your goals.

We're not mental health therapists, but living with chronic pain often goes deeper than a typical physical therapy visit. We make room for how this pain affects your confidence, your relationships, and your sense of self, because many men have nowhere else to bring these conversations.

Nothing you share with us is off-limits. We treat pelvic pain and hear these stories every day, and there's nowhere else we'd rather be, because real healing starts with feeling safe enough to talk about all of it.

Pelvic health conditions often go hand in hand

Chronic pelvic pain often overlaps with urinary symptoms, sexual health concerns, and other pelvic floor issues. Understanding how these connect can help you recognize your own experience and find care that actually fits.
